Go to the nearest MM Mega Market store (find here).
Ask the receptionist for “MEMBERSHIP CARD REGISTER FORM”.
Fill all needful information in the form (ask the receptionist for help if having any queries). Customers will be asked for some required documents (see below) to complete registration.
The receptionist then gives you a temporary membership card (made of paper). With this one, customer should use to buy goods first and receive invoice from cashier after payment. Last, customer should go back to the reception with the invoice and get official membership card (made of plastic).
* Business company/ organization:
Copy of business licence or investment licence (notarized or with company stamp).
Copy of notarized certificate of tax registration (note: do not provide this paper if business licence included tax code).
Letter of introduction (with company stamp).
Letter of confirmation of total official staffs (note: this paper is only used in case customers ask for more card holders as usual. Customers should create this paper by themselves because of not having any templates from MM Mega Market)
* Self-employed professional
Duplicate of business licence (notarized or enclosed the original copy to be collated).
Duplicate of tax receipt or certificate of tax registration (notarized or enclosed the original copy to be collated).
